If you are a parent or legal guardian seeking assistance for your child, please call (334) 306-7470 to schedule an appointment.
We ask that you bring a valid photo ID and proof of income with you at the time of your appointment, as well as identification for each child you are applying on behalf of. If you have access to a printer please print and fill out the forms below, and have them with you at the time of your appointment.
